Configurable Forms in OPSCOM: Build Data Collection Around Your Organization

Configurable forms in OPSCOM let you collect what your organization needs to know without building a custom tool. You define the form, the people who need to respond complete it inside the OPSCOM app, and the responses are collected and exported from the same platform that already holds your operational records.
The same module works as an integrated survey tool. Survey-style questions run inside OPSCOM, so you can gather information or feedback without a third-party survey subscription, and the answers sit alongside your operational records instead of in a separate account.
OperationsCommander (OPSCOM) includes Forms as part of the same platform that handles permits, enforcement and incident management, so a form submission sits alongside the rest of an organization’s operational records rather than in a standalone tool.
This article covers what the Forms module does for your organization, what changes when collection moves into the platform, and how the same capability extends beyond parking.
What does the OPSCOM Forms module do?
Forms turn a request or a set of questions into a structured record. You decide what the form asks and how it is laid out, the people who need to respond complete it in the OPSCOM app, and every submission arrives as one entry rather than as an email someone has to re-key.
Its purpose is to replace a paper or email process with digital collection: one record per submission, tied to the submitter’s verified user profile (a registered user with a verified email address). That is the difference between a form and an inbox full of replies, and it is what makes the responses usable, because you can see who responded and not only how many replies came back.
Built around your process, not a fixed template
The questions and the layout are set by the administrator, so the form follows the way your organization already works instead of forcing the process into a fixed template. When the process changes, the form changes with it, and there is no custom development and no separate tool to keep in step.
One record per submission, inside the platform
The record exists from the moment the form is completed. You can see the people who responded and the information they gave, export the data as a spreadsheet for reporting or offline records, and retire a form when it is no longer needed while the entries that came through it stay saved for reference.
None of that record lives in a mailbox, a shared drive or someone’s personal spreadsheet, and none of it has to be re-entered before it is usable. When you need a variant of a form, it can be duplicated and adjusted rather than rebuilt.
Your team controls who can see and change the data
Access to forms is permission-based. Only administrators with the right permissions can create or change a form, and seeing the results is a separate permission, so the people who build forms and the people who read the submissions do not have to be the same group.
Reviewing those permissions periodically is a documented best practice, and it is what keeps an active form from being changed by someone who should only be reading it.
Can OPSCOM forms be used for more than parking?
Yes. A form is defined entirely by the administrator, so the module is not limited to parking data. The questions and the layout are whatever the organization needs, and the same Forms module can carry data collection for other parts of the operation.
Examples of what an admin-defined form could carry include a municipality collecting dog licence applications or burn permit requests, or a campus managing event registration. These illustrate the kind of request a form can hold; they are not a limit on the module.
Designed so people actually complete them
Most people complete a form on a phone, so brevity matters: a short, clearly structured form with logical questions gets finished, and a long or confusing one does not. Because you set the questions and the layout, you can keep the form to what you actually need.
Where forms fit in the OPSCOM platform
Forms are one capability of the same OPSCOM platform that connects parking management, enforcement and incident management on a single database. A submission recorded through Forms is part of that operational record rather than a file that lives somewhere else.
